We reserve the right to charge a $50 fee for missed appointments or late cancellations without adequate notice.
$30 Private Script Fee for any scripts requested outside of office hours or outside a 30-day window post your most recent appointment. (This is excluding any specialist medications such as Xolair, Nucala or Dupixent.)
All patients not seen for two years will be charged the New Patient Fees.
